An unattached patient is someone without a primary health care provider (physician or nurse practitioner) anywhere in Ontario and who is living within our catchment area.

1. Register with Health Care Connect at HealthCareConnect Online
2. Complete a Health Questionnaire to be placed on the clinics wait list Health Questionnaire 2018 (email your completed Health Questionnaire through our website at Document/Photo Upload or mail to; P.O. Box 870, Haliburton, ON K0M 1S0, Attention: New Patient Request)
