 REGISTERED DIETITIAN – 0.5 FTE



The Registered Dietitian (RD) is a specialist in nutrition, and responsible for the nutrition care of patients identified as moderate to high risk for chronic disease, or patients living with chronic disease. Working in collaboration with an interdisciplinary team, the RD will share expertise and knowledge of nutrient care with patients and their families, as well as other health care professionals.

Duties and Responsibilities

  • Assess patient nutritional status by gaining an understanding of food habits and preferences (socioeconomic, psycho social and cultural background)
  • Establish goal oriented treatment interventions with patient and relevant others
  • Provide family patient education and provide appropriate support to address nutritional needs
  • Develop care plans and monitor the effectiveness of dietary changes
  • Specialize in the nutritional management and prevention of chronic diseases
  • Supervises and trains dietetic interns, nutrition and medical students on placement at the FHT
  • Coordinate with other staff to ensure optimal delivery of service
  • Update, develop and evaluate patient education materials and group sessions on an on-going basis
  • Participate as an active member of the health care team through multidisciplinary patient care consults, quality improvement and evaluation activities; adjust programs and services accordingly
  • Become aware of community resources available to patients and refer appropriately
  • Completes quarterly and annual reports to Ontario Health based on requirements.

Education and Experience

  • Baccalaureate of Science in Nutrition or equivalent
  • Current registration with the College of Dietitians of Ontario required
  • Current membership with Dietitians of Canada Association, including professional liability insurance program
  • Strongly recommended to become a member of the Primary Care Dietitians Association
  • Basic CPR certification
  • Current Ontario Drivers’ License
  • Demonstrated experience in Community and/or Primary Care Setting

Skills/Abilities

  • Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery.
  • Superior leadership, organization, research, evaluation, time management, communication and interpersonal skills.
  • Proficiency in the use of the clinical information systems/electronic medical records
  • Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences.
  • Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient.
  • Excellent verbal and written communication skills
  • Exhibit an ability to be open and non-judgmental
  • Knowledge of the goals and structure of the organization, the policies and procedures and the programs.

 RURAL FAMILY MEDICINE PHYSICIAN – 4 POSITIONS AVAILABLE

We are a group of 8 physicians practising family medicine in the beautiful Haliburton Highlands and Minden area. We are also affiliated, and work collaboratively, with the Haliburton Highlands Family Health Team. We currently have a roster of 9,770 patients.

Some of our physicians are part of the staff of our local emergency department. We provide regular care of hospital in-patients, provide hospital on-call coverage, offer care to patients in LTC facilities (including on-call), and care to patients in hospices or a palliative care unit.

We are looking for a physician or physicians who would be interested in working with a collaborative team of health care professionals. We are a teaching site and have a full rotation of residents from the University of Toronto, Queen’s, McMaster and other. Compensation is based on a blended capitation model as a physician belonging to a Family Health Organization.
