Anticoagulation Monitoring
Many medical practices throughout the world have moved towards INR management programs overseen by pharmacists and registered nurses. At the Haliburton Highlands Family Health Team and the Family Medical Centre, our RN’s will monitor and make necessary and timely medication changes according to client’s INR.
Heart Health Workshop
This is an educational workshop about blood pressure and cholesterol. The information is presented by both an RN and a Dietitian.
Cardiac Education
Six weekly educational sessions to assist clients after a cardiac event/intervention. Focuses on education about heart health, medications, nutrition, risk factors, exercise and stress.
Haliburton County Stroke Support Group
This support group meets every 3rd Thursday of the month from 10:00-12:00 at the Fireside Lounge at Hyland Crest in Minden. This support group is for stroke survivors and/or caregivers.
Living With Stroke
Eight weekly interactive education sessions for clients living with a mild to moderate stroke plus their caregiver. Supported by the Heart and Stroke Foundation of Canada.
Vascular Health Clinic
Three individual sessions focusing on understanding vascular disease, healthy eating and physical activity.
