Policy for Patients Wishing to Switch Care Providers
All physicians and allied health professionals at the Haliburton Family Medical Centre and Haliburton Highlands Family Health Team work as a collaborative team. We combine our collective knowledge and regularly consult other physicians and allied health professionals in our office when medically warranted to provide patients with the best care experience possible.
Because of this team approach, patients wishing to leave their primary care physician will be leaving the team.

What are uninsured services?
Uninsured medical services, are services that are not covered by the Ontario Health Insurance Plan, and may be charged directly to the patient (or third party). Examples include copies and/or transmission of medical records, completion of forms (physicals for school, camps, pre-school, daycare, university and other educational institutions), back to work notes, sick notes, insurance certificates, driver’s medicals, aviation medicals, etc.
Please make sure before any appointment, that staff are aware of any forms that you require to be completed. Staff will let you know the cost ahead of time. Payment is due at time of visit.

How do I renew My Health Card
Please see the link below, or visit a ServiceOntario centre, for instructions on renewing your Health Card. Your Health Card must be valid and be presented at every visit to the clinic.
