A Registered Nurse will work with you to manage chronic diseases including vascular disease, metabolic syndrome, and breathing disorders.
5. Healthy Kids
A Registered Nurse will support families in understanding and maintaining their children’s health through education, consultation and will facilitate referral to other community services.
6. Social Work Services
A Social Worker will work with you to support you and link you to community services.
7. Nutrition Counseling
A Registered Dietitian will work with you to understand and practice health eating habits for you and your family.
8. Chiropody Services
A Chiropodist will work with you to treat foot problems.
